Please note that no order can be processed without a check properly made out and stapled to this form. If you choose to have your CDs mailed to you, note the addtional cost (per CD) for the mailer and postage.

If there are no delays in the engineering, the CDs are normally ready some time in August following the school year they represent. If yours are to be mailed, you will probably get them before the end of August. Otherwise, you can pick them up in September when school starts.
